Every active node on PET is not necessarily significant and needs to be evaluated for calcification / high density - features that suggest old healed disease
This post is really written out of frustration. As the video shows, I recently came across two cases where, just because the nodes were active on PET/CT, the patients were sent for guided biopsies. You can't forget first principles of CT scan when interpreting PET/CT scans.
